Patient Information

Your experience with the office begins and ends with our front office staff. They are committed to deal with patients in a professional, caring and efficient manner. Our staff is able to assist with scheduling your appointments as well as any testing that the doctor has ordered.  Our multilingual staff is fluent in Farsi, Spanish, Russian and English they are committed to be your advocates during for the entire appointment process.

Upon arrival you will assist us in creating your medical profile, we ask that you complete all patient form and responsibility forms. Insurance companies and government payors such as Medicare require that we provide them with up to date and accurate information. We certainly appreciate your informing the front office staff of any changes of address, phone number, emergency contacts or insurance status that may occur in the future.

Providing an email address will allow us to provide you with our medical newsletter that will provide current medical information that you will find of useful.

If schedule conflicts arise that do not allow you to make your appointment, we ask that you notify the office staff at least 24 hours prior to that appointment so we can add other patients to the schedule who may be wait-listed for appointments. 

Please Note: If 24 hours notice is not provided a $50 “No-Show” fee will be assessed for office visits, $100 for ECHO and vascular testing and $250 for stress testing.
